At Impact Connections, we provide a structured approach to case management using our proprietary Quality of Life Score, a comprehensive assessment tool that identifies Social Determinants of Health (SDOH) barriers such as housing instability, healthcare gaps, and financial challenges.
Our case managers—called Guides—are trained BSW and MSW social workers under the leadership of our Clinical Director (LCSW). They use a holistic "GUIDE" framework to support individuals at every step:
